Antidepressants, or selective serotonin reuptake inhibitors (SSRIs), are commonly prescribed for major depressive disorder (MDD) and anxiety disorders. To better understand the long-term safety and efficacy of antidepressants, the aim of this systematic review was to evaluate the evidence on the long-term safety and efficacy of antidepressants. We searched the MEDLINE (1966–December 2018), EMBASE (1980–February 2018), EMBASE (1948–December 2018) and the Cochrane Database of Systematic Reviews (aticcra Rx 2018) up to January 1, 2019. We also searched the reference lists of included studies for the primary outcomes evaluated in the analysis and our primary studies to identify eligible studies. We then compared the long-term safety and efficacy of antidepressants with standard of care in the two groups. We included only those studies comparing antidepressants with standard of care in the two groups, and the outcomes of interest were HSDL, remission or remission rates.
We used a search strategy that was initially developed by the Cochrane Central Register of Controlled Trials (CENTRAL) database. The Cochrane Library was first identified as a literature search and then reviewed by the PRISMA guidelines for publication of studies in English. We excluded studies that did not use any search terms or that did not have an active inclusion/exclusion criteria.
We extracted the relevant literature and selected studies through a PubMed/MEDLINE and EMBASE search. We extracted the following: (1) all studies comparing antidepressants with standard of care in the two groups; (2) all studies comparing antidepressants with standard of care in the two groups; and (3) the outcomes of interest. We also excluded studies with the following inclusion/exclusion criteria: (1) reviews and meta-analyses reporting on antidepressants, and (2) case reports, meta-analyses or case series reporting on antidepressants, and (3) unpublished studies.
We assessed the quality of the data using the Cochrane Collaboration's tool. The quality of the data was assessed through the International Index of Efficacy (IIEF) and the Global Burden of Disease tool. We selected studies based on a combination of the following: (1) study design, (2) sample size, (3) sample size, and (4) sample size; and (5) design, sample size, and (6) study type and population. We also reviewed the characteristics of the included studies, their risk of bias, and the quality of the evidence.
The risk of bias of the included studies was assessed using the Cochrane Collaboration's tool. The risk of bias for each included study was assessed using the Grading of Recommendations Clinical Studies-Fourth Year (GROCA-4) scale. Studies were categorized as low risk of bias due to the inclusion of small studies; low risk of bias due to the exclusion of larger studies; and high risk of bias due to the exclusion of small trials. We excluded studies that did not report the total number of participants, patients with a long-term follow-up period, and/or those that did not report the number of participants. Two authors independently evaluated the risk of bias using the Cochrane Collaboration's tool. The quality of the studies was assessed using the GROCA scale. We also examined the data of studies using the BDI-IIEF.
